Age-Related Macular Degeneration (AMD): The Evidence-Based Supplement Protocol

Age-related macular degeneration is the leading cause of blindness in adults over 65 in developed countries. The AREDS and AREDS2 trials remain among the cleanest and most influential supplement trials in medicine, producing a specific formula that slows AMD progression. The formula matters more than any individual component.

The AREDS2 Formula — The Core Protocol

AREDS2 (2013) tested modifications to the original AREDS formula in 4,203 adults at risk of advanced AMD over 5 years. The final recommended formula: lutein 10 mg + zeaxanthin 2 mg + vitamin C 500 mg + vitamin E 400 IU + zinc oxide 80 mg (later modified to 25 mg in some formulations) + cupric oxide 2 mg. This combination reduced progression to advanced AMD by ~25% over 5 years in adults with intermediate AMD or advanced AMD in one eye. See our lutein/zeaxanthin piece.

Lutein + Zeaxanthin — The Macular Pigment

Lutein and zeaxanthin are the carotenoids deposited in the macula where they filter blue light and quench reactive oxygen species. AREDS2 specifically replaced beta-carotene with these because beta-carotene increases lung cancer risk in smokers. Effective doses are 10 + 2 mg daily.

Zinc — The Active Mineral

Zinc is essential for retinal pigment epithelium function. The original AREDS formula used 80 mg of elemental zinc daily — substantially above what most general-population supplements provide. AREDS2 confirmed efficacy at 25 mg as well, with potentially better tolerability. Copper (2 mg) is essential to prevent zinc-induced copper deficiency. See zinc form piece.

Omega-3 — DID NOT Help in AREDS2

AREDS2 specifically tested omega-3 (EPA 650 mg + DHA 350 mg daily) and found no benefit for AMD progression. The trial was well-powered and the result is robust. Omega-3 may be useful for other reasons but not for slowing AMD specifically.

What NOT to Take

Avoid beta-carotene if you are a current or former smoker — CARET and ATBC trials showed increased lung cancer risk. AREDS2 specifically removed it. Skip "eye health complex" formulas with subclinical doses of 12+ ingredients — they don't match the AREDS2 trial-validated formula. Avoid bilberry as a primary AMD intervention — observational only. Don't substitute generic carotenoid blends for the specific lutein 10 + zeaxanthin 2 mg dosing — different formulations have produced very different effects.

How to Run the Protocol

Confirm intermediate-or-advanced AMD diagnosis with retinal specialist. Smoking cessation is the single largest preventive intervention. Start the AREDS2 formula (lutein 10 + zeaxanthin 2 + C 500 + E 400 + zinc 25 + copper 2 mg daily). Annual ophthalmologic follow-up.
